Mount Pleasant Essay

Louise Buxton’s Mount Pleasant is a very uniquely written short story, where we experience a normal day from a child perspective. It’s a modern story from 2005. The story is told in first person singular, where the narrator is a young girl whose name is Elizabeth. †(†¦) like the glue wee peel off our hands after arts and crafts hour at school with Mrs Kelly.† (p.2, l.16-17) Based on the information we know that Elizabeth goes to school, but also that she cannot be more than 6 to 8 years old. When she finds the old photograph of a boy which is a 9-10 year in a costume, we are told that she is not quite as old as he is. We know that she is a young girl, judging by her verbal skill and behavior. â€Å"With the sun in my eyes, I couldn’t see my mammy’s face, not even squinting with both arms above my head and my head tilted†. (p.2, l.22-23) The language is very descriptive and it seems as if one were looking through the eyes of a child. The short story varies from other traditional first person stories by putting the focus on a child and what a child does. Elizabeth’s focus is on everyday play, and those days are to her full of adventure and close friendships: †Lena, Little Fran, Wilf and me – playing for hours at house or working at some theatrics†. (p.5, l.152-153) Or where there is a break with everyday routines. For example, when Lena and Elizabeth discovers a business with unlimited possibilities in the form of candy and ice, until the mother puts an end to the adventure: †(†¦) raspberry ruffles and chewing nuts and chocolate raisins and liq-uorice sticks and ice cups and strawberry boot lace. We put them on Mammy’s account at first but Mammy found out after she got given a bill (†¦).† (p.4, l.92-94) The children also play ghost games, where they attempt to frighten one another by dressing up in linens and pursue one another. In several cases reveals the narrator herself as a child by acting irationaly. When Lena and Elizabeth lie in their beds and think they can hear ghosts, they pull the sheet over their heads and lie still. The reaction shows a childish logic. If they cannot see the ghost, then the ghosts cannot see them: †If I don’t move, they can’t get me.† (p.5, l.147-149) Since the perspective is a child’s, it is particularly interesting to see where the child’s perception of situations and episodes differs from that of an adult. For example, when Elizabeth thinks that the mother reknits the nuns clothes, because the  mother is good at knitting and can do it better: †(†¦) and mammy isn’t proud, she always takes them – and unravels them and knits them again, better, because she’s a good knitter, my mam†. (p.4, l.87-89) The reason is more likely that the mother does not want people to know that they have gotten the clothes from the nuns. The narrator being a child has the effect that we as readers several times pause to think about what it means. At several places in the story where Elizabeth tells about an episode where the reader has to interpret the situation, to determine whether it’s a ghosts or just the narrator’s childish imagination. It’s for example unclear in Elizabeth’s depiction of how the photograph ends up on the mantelpiece again and again: †But somehow it found its way back on top of the mantelpiece.† (p.4, l.102) Or where she feels like there is somebody sitting on the bed and watching them. The fact that the narrator is a child is very easy to see by analyzing the language. She mentions her parents with expressions such as â€Å"mammy and â€Å"daddy†. The little brother, George William she calls â€Å"babby†. The language is characterized by an absence of literary language. An example of this is where she says that their hands are â€Å"(†¦) lickity-stickity†. (p.5, l. 121-122) She invents her own expressions, which results in the story being poor in traditional imagery like comparisons and metaphors. The imagery used is mainly related to the child’s imagination. For example when she feels uncomfortable in her clothes she says: † I’m all blowed up like an old frog.† (p.2, l.12) Another interesting thing is how the sounds in the house are described. Elizabeth perceives the sounds caused by the wind to be scary, and describes it with the use of personification. The short story takes place in an environment that Elizabeth knows well, which is at their house, Mount Pleasant. We don’t hear much about the world outside the neighborhood, or thoughts that exceed it. This is of course because she is a child who is not involved in it. â€Å"We know the roads around Mount Pleasant pretty well now. And everybody knows me and Lena.† (p.4, l.83-84) The tittle of the short story Mount Pleasant, which is the name of their house, indicates how safe and merry their life is, and stands as a stark contrast to the spookiness. Mount Pleasant can be seen as a symbol of childhood joys and play. Her childhood eventually gives in to a more mature and realistic  perception of reality. When Elizabeth throws the photograph in the fireplace, it seems as if she actively tries to fight her fear. She might think that the deceased boy had something to do with the ghost. She goes to bed, and the tension builds up and gets stronger and stronger until she finds out that there is nobody there at all. Elizabeth takes a small step towards becoming an adult; she finds out that there are no connections between the photo, the scary sounds and the deceased boy.

Bill Gates Criticism: With his great success in the computer software industry also came many criticisms. With his ambitious and aggressive business philosophy, Gates or his Microsoft lawyers have been in and out of courtrooms fighting legal battles almost since Microsoft began. The Microsoft monopoly sets about completely dominating every market it enters through either acquisition, aggressive business tactics or a combination of them. Many of the largest technology companies have fought legally against the actions of Microsoft, including Apple Computer, Netscape, Opera, WordPerfect, and sun Microsystems. Why do I like Bill Gates: I think Bill Gates represents the best of us. – its not just what he’s doing but how he thinks about what he’s doing. He’s a curious geek. He wants to find interesting problems to solve. He believes that smart, self-motivated people working together can make a difference. Bill Gates reflects the best qualities of a generation that has grown up finding the innovative ways to apply science and technology to impact our everyday life in mostly positive ways. Gates made an important and potentially difficult transition at age 52, leaving Microsoft as CEO and devoting more of his time and energy to the Bill and Melinda Gates Foundation. It’s a shift in focus, moving from defining strategy for Microsoft to a broader strategy for improving the lives of the world’s poor. He is a self-made American who has matured into a role model and leader. He is thoughtful and tactful where a younger version would have been brash and impetuous. Like Windows, improvement for Gates has required multiple iterations but the insistence on getting it right won out eventually. The newest release of Bill Gates is the best yet. He’s not just analytical when he talks about improving education. He appears to be moved while describing his interaction with highly motivated teachers who see their profession â€Å"as a higher calling. † He highly recommends â€Å"Big History† a series of lectures by David Christian, available through â€Å"The Teaching Company. † It`s really very depressing when we learn that how American executives have brought ruin to American business and our economy. They aren’t leaders worth following. Gates is different. He deserves genuine admiration, in my view. He’s more than a technologist. He’s both a realist and an optimist. He’s become a world leader worth listening to.

100 Years

100 years from now, many dimensions of human life will change dramatically. Science, medicine, and government will certainly change, and confidently for the better. The Homo sapiens species will advance and how things are now will be ancient history only to be taught through text in school, with many details of previous life long forgotten. The United Nations will develop a way to eradicate war and dispute through a full proof â€Å"Universal Problem Resolution Plan†. Therefore the world will finally be at peace, work as one, and the once poverty-stricken will flourish and starvation will lie in the individuals will to eat, not the â€Å"cards they’re dealt†. With war and fighting in the past the world will place all the dollars once used in military and armed forces into healthcare research and education. Doctors will have fashioned and perfected organ growth and the diseases we now fear such as A. I. D. S and Cancer will be thought of as a common cold due to easily accessible vaccines and remedies. Life will be different, but my outlook and attitude towards life will remain the same. I would wake up every morning happy that God gave me another day with my family and friends. I would wake up ready to make a difference and play my part in this world. I would not be raised from sleep by an alarm clock, but rather a microchip implanted behind my ear that told my brains receptors it was time to get up. The chip would also have my itinerary for the day and any important events I had planned. Prior to going to bed I would fill out what I wanted the chip to remind me on my phone and simply hold the phone beside my head so the chip could scan the information, store it, and later activate at the given time. Phones would still be referred to as phones but the technology that followed them would be phenomenal. To answer a call, you simply say â€Å"answer† or whatever you have programmed as your command to connect. The phone would then bring up a holographic display of the caller so interaction as well as speaking would take place. By the time I was awake and teeth had been cleaned to spec, I would go to my virtual wardrobe selector (VWS) and decide what to wear. The touch screen display allows me to choose what I would like to wear and delivers it to me with a solar powered track system. At this point 95% of the worlds power supply is solar and wind derived. Upon leaving the house, I would scan my thumb and the house would be locked and secure until I returned with virtually no way for trespassing or criminal mischief. Once I got in my car and turned on the ignition, powered by voice activation, along with everything else, I could drive to work myself (manually) or have the car drive itself there. The vehicle would be able to do this using satellite navigation, traffic and pedestrian observation sensors, and lightweight magnets in the body and the road. Traffic accidents and deaths would reduce by 200% after this technology was perfected. Although everyday life would be much easier, work would be intense as ever and the demand for good jobs would be outrageous. As a well renowned and highly sought after attorney I would have no problem with work, but still worked hard to keep my clients and those within my firm satisfied and content. Life would definitely be different, but still crazy. To keep from losing my sanity in the â€Å"once crazy, and still crazy† world I would surround myself with the ones who love me and love them just as much. I would continue to have faith in the Lord throughout my life and place nothing above him and his word. Technology would be great and the safety that came with it would allow most humans to live past 100 years old. In fact, I am 118.

Some work under me with finance and we have to cooperate with nurses, physician assistants, doctors, maintenance work, etc. Under the terms of the prime minister, he calls for corporate responsibi lity of health care as well as measuring progress and outcome. In my particular area of administration this is what I focus on because it is necessary as a leader to take responsibility for ones actions as well as being aware of the progress being made by not only my department, but also by the university (Department of Health and Social Security, 1972). Because each group focuses on a different aspect of the work we do at the veterinarian university, sometimes when we are forced to work together on a multidisciplinary problem, each person focuses on solving it from his/her perspective rather than trying to find a unified solution. Also, other problems with diversity range from the decreasing of females in the workforce and the increase in males. This sexual inequality sometimes creates tension not only among individual apartments, but also among our workplace as a whole. As a leader, I have to be able to deal with these problems in diversity quickly and efficiently while using soun d judgment and making sure that we stay functional. The first important component in the scheme of diversity is understanding the protocol and policies in which the nature of my work focuses on. The healthcare world is constantly updating and changing. As a leader, it is my responsibility to be adaptable and get these changes out to my subordinates so they are completely informed when they are dealing with their work. This also includes being aware of policies that are not only made by national standards, but also by our work center. By being proactively involved in the creation of better and more incorporating policies, differences in diversity as well as making work more efficient and effective can result (Hughes, 2005). In addition to being adaptable, the nature of my work requires me to be proactive in change. This occurs by me using my political and social influences and knowledge in order to shape policy (Thomas et al., 2004). This requires me to have knowledge of how to know what policies I want to effect as well as the tactics to go about doing it as well as who it will effect (Antrobus et al., 2009). This requires knowing what my vision of my department is and trying to influence it in such a way that I will see the desired changes. While diversity can promote development within the workplace, sometimes it can hinder it was well. In particular, my role as a manager at a clinical hospital often relies on the interaction with not only my staff, but that of others as well. Case Study On A Patient With Heart Failure Mr. SB, 60-year-old male is a retiree and was admitted to the hospital accompanied by his daughter. He is 100kg at a height of 180cm so his calculated body mass index (BMI) was 30.9 indicating that he was overweight. When admitted, patient was complained of shortness of breath for 2 weeks and was worsening on the day of admission. Besides, he also experienced orthopnea, fatigue, paroxysmal nocturnal dyspnea and leg swelling up to his thigh. Mr. SB was admitted to the hospital for to the same problem last year. Mr. SB had known case of heart failure since 3 years ago and he had also diagnosed with hypertension for 5 years. Before admitted to the hospital, patient was taking frusemide 40mg, aspirin 150mg, metoprolol 50mg, amlodipine 10mg, and simvastatin 40mg for his hypertension and heart failure. Patient does not allergic to any medication and he does not take any traditional medicines at home. His family history revealed that his father had died of ischemic heart disease 4 years ago while his brother has hypertension. As for his social history, he smokes 2-3 cigarettes a day for 35 years and the calculated smoking pack years was 5 pack years. Besides, Mr. SB also drinks occasionally. On examination, Mr. SB was found to be alert and conscious but he was having pedal oedema up to his knee. Besides, the patient was noted with bibasal crepitations with no rhonchi. His body temperature was normal. However, his blood pressure was found to be elevated upon admission with a record of 159/100 mmHg with an irregular pulse rate at 85beats/min. His echocardiogram showed that he had left ventricle hypertrophy while chest X-ray was conducted and revealed that the patient had cardiomegaly. Lab investigations such as full blood count, liver function test, urea and electrolyte test and cardiac enzyme were done upon admission. His creatinine concentration was found to be 143 µmol/L. Therefore, the calculated creatinine clearance was 68.8ml/min. Besides, there was also blood found in the urine and the echocardiography showed that the patient has sinus tachycardia. In addition, ECG test was performed on day 1 and the result indicated that there was a T-wave inversion. The patients INR was 1.04 which was lower than normal while APTT was found to be slightly higher (59.4 seconds). Mr. SBs random blood glucose was found to be normal during his hospitalization. Mr. SB was diagnosed with congestive cardiac failure (CCF) with fluid overload. The patient also suffered from hypertension. The management plan included intraveneous frusemide 40mg twice daily, aspirin 150mg once daily, simvastatin 40mg once at night and ramipril 2.5mg once a day. Besides, patient was asked to restrict his fluid intake to 500ml per day and oxygen therapy was given to patient at high flow using a face mask when patient experiencing shortness of breath. As for his clinical progression, on day 1, the patient was complained of shortness of breath, leg swelling and orthopnea. Enchocardiogram showed that he had cardiomegaly. Treatment of CCF was given. Throughout the stay in the hospital, Mr. SB had responded well to the heart failure therapy as there was no more complaint of chest pain or shortness of breath on day 13 and his pedal oedema had gradually improved. However, patients blood pressure throughout day 1 to 9 was fluctuating between the range of 102/67-160/100 mmHg and therefore, hypertension treatment was given and blood pressure on day 10 onwards had been seen fell within the normal range. Furthermore, Mr. SBs renal function became progressively worse from 143 µmol/L on admission to 175 µmol/L on day 11 and the calculated creatinine clearance on day 11 was 56.2ml/min. 2. Pharmacological Basis of Drug Therapy 2.1 Disease Summary Congestive cardiac failure (CCF) is a complex syndrome that is usually caused by the inability of heart to pump sufficient blood to meet metabolic needs of body during exercise. It is more commonly known as heart failure38 and it can affect either left or right ventricle or both39. The risk factors predisposing one to heart failure are obesity, high blood pressure, diabetes, and smoking. Heart failure is commonly characterized by typical signs of fluid retention with symptoms of breathlessness, fatigue, paroxysmal nocturnal dyspnoea, and reduced exercise tolerance39. CCF is a common disease which affects approximately 1-2% of the general population in developed countries1. Prevalence increases with age especially those aged above 75 years where the prevalence of CCF could be as high as 10%2. In addition, men are prone to getting heart failure as compared to women1. Each year, there are about 1-5 new cases of CCF per 1,000 population and it also increased with age40. In United Kingdom, the incidence of CCF is about 0.02 cases per 1000 per annum between the ages of 25-34. However, the incidence increased to 11.6 cases in those above 86 years old1. The prognosis for CHF is relatively poor. Approximately 40% of individuals with CCF die within a year after diagnosis3. There are many causes of CHF but the most common underlying causes are heart attack, coronary heart disease, and high blood pressure. Others such as cardiomyopathy, valvular heart disease and diabetes may also precipitate heart failure4. An early diagnosis of CHF is often based on the signs and symptoms which the patient is experiencing5. Other tests are needed to confirm or rule out the diagnosis. These include chest X-ray examination, physical examination, electrocardiograph (ECG), echocardiography and exercise testing. The severity of heart failure can be classified according to the New York Heart Association (NYHA) classification system. This system consists of four classes which relate patients symptoms to physical activities and quality of life. Table 1: New York Heart Association (NYHA) Classification5. Class Patient Symptoms I (Mild) No symptoms with ordinary physical activity (walking and climbing stairs) II (Mild) Slight limitation of activity with dyspnoea to severe exertions (climbing stairs or walking uphill) III (Moderate) Marked limitation of activity. (unable to carry on physical activity without discomfort) 2.2 Drug pharmacology in treatment of congestive cardiac failure Chronic cardiac failure should be treated immediately once it is diagnosed. The goal of treatment is to improve patients quality of life by alleviating the symptoms, improving exercise tolerance, preventing the progression of myocardial damage as well as reducing hospital admission and mortality. Angiotensin-converting enzyme inhibitors (ACEis) ACE inhibitors are considered as first line therapy in patients with CCF5. They bind to and inhibit angiotensin converting enzyme which subsequently inhibit the action of angiotensin I. As a consequence, the production of angiotensin II is prevented. Angiotensin II is a potent vasoconstrictor which has a direct action on kidney to stimulate the secretion of aldosterone and antidiuretic hormone (ADH). This will cause sodium and water retention. Other common side effects include hyperkalaemia, profound hypotension and gastrointestinal disturbances15. ACE inh ibitors are contraindicated in patients with renal impairment even though some studies have shown that they have renal protective properties43. Example of ACE inhibitors are captopril, enalapril, and ramipril. The starting dose for ACEis should be low and the dose should be increased gradually to target doses5. Beta blockers Beta blockers used to be contraindicated in patients with CCF as it may worsen the condition of the heart due to its negative inotropic effect. Nowadays, beta blockers should be considered in all patients with heart failure unless contraindicated5 as they have been shown to reduce the mortality, hospitalization and the progression of heart failure7. Beta blockers should be introduced following treatment with ACE inhibitor once the patients condition is stable7. Only bisoprolol, carvedilol, and nebivolol are currently licensed to be used in the treatment of heart failure in UK8. Both nebivolol and bisoprolol are cardioselective where they act on beta ­1 receptors. On the other hand, carvedilol is a non-selective beta blocker9, 10. The mode of action of beta blockers in heart failure is poorly understood but the proposed mechanisms include antiarrhythmic action, anti-ischaemic action, and attenuation of cathecholamine toxicity as well as reduced cardiac modelling through blockade of sympathetic influences on the heart9. Besides, carvedilol has an additional antioxidant property which may be thought to slow down the process of atherogenesis by inhibiting the oxygen-free radicals11, 12. The starting dose should be low as high doses may worsen the condition of heart failure7. Over time, the dose of beta blocker should be gradually titrated upward if the patient is well tolerated until target dose is reached5. Diuretics Diuretics are often used to relief the congestive symptoms and fluid retention7. Hence, they should be used in heart failure patients with the symptom of oedema7. Frusemide, a loop diuretic is the most commonly used agent in heart failure. It is considered as the first choice of drug for the long-term treatment of CCF with the advantages of improves cardiac function, exercise tolerance, as well as symptoms of breathlessness and oedema13. The main site of action is at the thick ascending limb of the loop of Henle. Furosemide acts at the Cl- binding site of Na+/K+/2Cl- co-transport and as a result, sodium reabsorption is inhibited. Because of this, Shakespeare was forced to write on many different levels, the most sophisticated of which appealed to his more elite audience members, while the more straightforward and often more crude of which appealed to his less educated viewers, and the most universal of which still appeals to us. In act 3, scene 4 of King Lear, Shakespeare demonstrates the multi-layered quality of his writing in the conversations that takes place between King Lear, The Fool, Edgar and Kent. Shakespeare uses the language of Edgar, which is witty, crude, and a little bit shocking, in order to appeal to his lower class audience. However, at the same time, Shakespeare uses the entire scene to draw an extended metaphor between the inner turmoil of King Lear’s life, and the outer turmoil of the storm, a more subtle literary element that would have appealed to a person with a better education. This serious financial pressure is creating major stress for her and the family, as she is the only earning member. In addition, she is not coping well with the stresses and is struggling with the loss of significance suffered in the redundancy. At fifty years of age, her hope for another equally paid level of employment seems gone. At the session Theresa looked anxious, fidgety and had a dejected tone. My flaw is overthinking. Always over analyzing someone’s words or actions into what I assume they are meant to be.This major flaw of mine I believe is the cause of my insecurities. Over thinking has caused me to stress myself out over minor situations that could have easily been avoided. I developed early, very early, as early as 7 years old. In my young mind, I thought I was just like everyone else. That was until someone finally spotted it out, making m e aware of my abnormality. This then made me feel self-conscious about my physical appearance. For years to pass, I continued to believe that my abnormality was wrong. As if I was supposed to look like one thing but did not. I hit puberty before anyone of my age.I was the tallest amongst my friends and classmates and the most developed. Boys at my age did not look at girls that were â€Å"bigger† than them, or looked older than them. They were interested in girls that looked their age, which was only about 9 and 10. I did not take this as â€Å"I’m too good for them† or â€Å"They’re not on my level yet. † I took this as â€Å"boys don’t want me because I am ugly. † I would think that people only wanted to befriend me just to have a reason to laugh at me, or make me cry – considering I was a huge cry baby – I would think no one genuinely wanted to be my friend.We are all taught to watch our surroundings and who we allow in our lives. After all, you are a reflection of those that you surround yourself with. As I’ve grown up and have matured, a lot, I am still very self-conscious, but I choose not to let people know. All the hurt and agony I feel, I hide and keep to myself. It can hurt to hear the truth, the desire to know the truth but afraid of what the answer may be. This is where the overthinking plays in. The simplest thought, I over analyze and interpret in my own way and this can most likely cause me to come up with conclusions that were never discussed.These thoughts that I come u with tend to bring me down, making me feel worse than before. My mind is consistently in motion, I am always processing something. Without the peace of mind I desperately want, I will always scrutinize everything. This is a truly personal flaw that no one could ever point out, much more complex than any physical â€Å"flaw†. I have grown to not care what people have to say about what may seem as a flaw to them but I still undergo a personal struggle of attempting not to over analyze situations and allowing them to solve itself instead of making up a solution in my conflicted mind.
